WHAT WE PROVIDE

Central Sydney Community Nursing Service provides nursing care by Registered Nurses for people living and working in Central Sydney

The aim of this care is to promote good health and well being and also to support and encourage independence for people living at home

Services provided include:

¨Treatment and dressing of wounds

¨Medication Assistance

¨Management of bladder and bowel continence problems

¨Palliative care

¨Support for clients and carers who need care and support when someone is dying

¨Support for clients and carers who are coping with illness.

¨Support for clients and carers who are caring for someone with dementia

¨Help with organising personal care assistance

¨Health Promotion

Community Nurses have access to Clinical Nurse Consultants in areas such as:

¨Oncology/Palliative Care

¨Wound Care / Infection Control

¨HIV / AIDS

¨TB

¨Continence

¨Spinal Injuries

Clinical Nurse Consultants have an expert knowledge of the specialised needs of clients, and nurses may access their advice as part of your care. 

The nurses may liaise with and refer you to other organisations and health professionals as necessary.  These could include:

¨Aged Care Assessment Team

¨General Practitioners/Specialist Doctors

¨Palliative Care Nurses and Doctors

¨Home Care NSW

¨Community Aged Care Packages

¨Meals on Wheels

¨Respite Service

¨Ethnic Services (including interpreters)

¨Social Workers

¨Occupational Therapy

¨Physiotherapy

¨Mental Health Team

WHO IS ELIGIBLE

Those people residing in the boundaries of Central Sydney Area Health Service who require community nursing care.

Our office hours are  8am - 4.30pm

Monday - Friday

The service operates 7 days a week

Evening visits can be arranged but are subject to service availability

Clients can be linked to the After Hours Palliative Care Service, which provides a 24 Hour coverage.

A weekend service is available to clients based on their clinical need and treatment requirements.

HOW TO MAKE A REFERRAL
Contact the centre nearest you between the hours of 10am - 4 pm
Monday to Friday

When the nurse visits she or he will:

¨Discuss how often visits will be made

¨Plan your care with you

¨Inform you of your rights and responsibilities

¨Ask if there is someone else you consent to being involved in your care

 

ADDITIONAL INFORMATION

No Smoking:  In the interest of our nurses' well being, we ask you, your family or carer not to smoke whilst the nurse is in your home.

Dogs & Cats: Restrain any dogs and cats

Home Changes:  Sometimes we have to make changes to homes where we need to use lifting equipment or special aids. 

Where changes are required we thank you for your understanding and assistance
